AFib monitor / Description

This Wear OS app is running an algorithm for the detection of atrial fibrillation (AF) designed to operate on continuous photoplethysmogram (PPG) signals recorded using an optical sensor on the bottom of a wrist-worn device. Robustness against false alarms is achieved by the means of signal quality assessment.

In order for the application to work the "demo mode" should be turned off in the settings and the wrist-worn device must be fastened on the wrist. Pressing play/stop button starts/stops monitoring. The left button enters settings while the right button opens the heart rate chart. When monitoring is initiated the application checks whether the default optical sensor is available and in case it is not the application would either ask user to continue or quit the application. If user choses to log heart rate it will be displayed in the log screen (accessed by pressing right button). If atrial fibrillation function is enabled in the settings and it atrial fibrillation was suspected during the monitoring log would also include boundaries of the detected atrial fibrillation episode by marking it with blue lines (see screenshots). The wrist-worn device would vibrate three successive times (if this functionality was enabled in the setting) if the episode of atrial fibrillation was suspected and two successive times when it ended. The monitoring is stopped automatically when battery charge drops to 15%.

The "demo mode" is used to demonstrate the exact operation of the app. An irregular rhythm in that mode would cause the algorithm to detect atrial fibrillation therefore user can observe what would happen if atrial fibrillation was suspected during the usage of the app.

Some devices such as Fossil Gen 5 TicWatch E3 will have option named "Sensor mode" in Settings. The difference between "Mode 2" and a default "Mode 1" is the that "Mode 2" uses raw PPG signals in order to obtain heart rate values (calculated by the algorithm in the app ) and "Mode 1" uses pre-calculated heart rate values obtained from the watch (calculated by the firmware of the watch). Not all watches provide sufficient quality of a raw PPG signal therefore this mode is only recommended for such watches as Fossil Gen 5. If quality of the PPG signal was good "Mode 2" would provide heart rate values which are more suitable for the AFib detection algorithm than those provided by the firmware of the watch.

Although this AFib monitor was tested using clinical data keep in mind that it is not a medical-grade detector therefore it should only be considered as a recommendatory application and each suspected onset of atrial fibrillation should be considered with caution. Also keep in mind that different devices contain different optical sensors which are directly responsible for the quality of the signal. The quality of the signal also depends on how well the wrist-worn device is fastened on the wrist: it should be as close to the skin as possible however wrist should not be squeezed too much. If heart rate value is displayed on the top of the screen then the signal quality is acceptable you should also see the signal similar to that shown in the screenshots of the app.
